What insurances does Neurosurgical Associates participate with?

Below is a list of insurances that Neurosurgical Associates participates with as of 2/12/08. We will file your claims directly to your insurance company if you have provided us with complete insurance information. Please be sure to check with your insurance plan regarding referrals to ensure that you receive maximum coverage.

Effective June 1, 2010, general assistance medical care (GAMC) patients will no longer be allowed to treat at our clinic due to the recent change in MN Legislation.

How do I know if my insurance will cover my visit or surgery? How much will it cost?

Coverage varies with each insurance company, so please refer to your insurance member handbook or call your insurance company with questions about what’s covered.

The business office will offer an estimate for our surgeon's fee if requested.  Neurosurgical Associates is not able to give estimates for procedures that are not performed by our clinical staff such as facility charges, diagnostic imaging, labs or physical therapy. 

Any estimates given are a general quote of benefits and are not a guarantee of payment.  Actual payment is based on your insurance benefits.  You should always check with your insurance company regarding your specific benefits.

What are my options if I am experiencing financial hardship?

 

 

If you are experiencing a financial hardship, please call the business office at 612-879-7190 to discuss your specific situation.
 
Neurosurgical Associates, Ltd has monthly payment plans thru MedCredit, discounts on medical bills and Hardship Assistance Programs.
 
If you cannot pay your portion of the medical bill in full, the business office will work with you. We will help you set up a payment plan thru MedCredit. If you do not have insurance or money to pay your bill, there may still be options available for you.
 
You will be asked to apply for Medical Assistance or Minnesota Care before you can qualify for our hardship program. This is so we can conserve funds for people with no other source of payment for their medical bills.
 
To help determine if you qualify for a hardship discounts, Neurosurgical Associates, Ltd:
  • Uses income guidelines issued by the U.S. Department of Health and Human Services;
  • Considers all of your financial assets and family size; and
  • Requires your provide the personal and financial information we ask for
 
You will continue to receive a bill until we decide if you qualify for the program.
 
We are committed to helping you and your family receive the medical care you need regardless of your current financial situation. We cannot help without hearing from you. If you need special assistance, please let us know so we can help find a solution.
 
 
Uninsured Discount Program
For new patients who are uninsured, Neurosurgical Associates, Ltd requires a down payment of $200.00 for consults. Established patients will require a down payment of $100.00 for each visit.
 
If surgery is required, a down payment of 50% will be required.
 
 
Special circumstances (individual, case-by-case basis)
Special circumstances will be reviewed on an individual case by case basis. In addition to the financial information provided, there may be circumstances where a patient experiencing a catastrophic event puts them in a devastating financial position whereby Neurosurgical Associates, Ltd will determine how to best assist patient in resolving balance owing. Please contact the business office to discuss your options for Hardship Assistance
 
MedCredit Financial Services
MedCredit Financial Services provides monthly payment plans to patients who can’t afford to pay their medical bills. Immediate financing approval for new accounts with NO CREDIT CHECK required. They can consolidate all medical expenses from participating providers (any health care provider that offers MedCredit Financial Service to patients) so that the patient only has one monthly payment. Once they have a MedCredit Financial Services account established, patients can add on any additional medical expenses for themselves and their family.

Will the clinic bill my insurance company, worker's compensation or medical liability carrier for my visit?

Yes. To make sure we have the right billing information, we will ask to see your insurance card at every visit. We also will ask you to verify your name, address, Social Security number and employer.

If your claim is for a work-related injury, you will need to provide the necessary information to bill the liability insurer. To comply with State of Minnesota guidelines, we may have to reschedule your appointment if we have not been able to verify your worker’s compensation insurance prior to your appointment.

If insurance information is not available or not provided to us, the account will be billed directly to you.

Please remember that you are ultimately responsible for the charges on your account for treatment you received. Neurosurgical Associates, Ltd will not become involved in liability disputes.

When will I receive a bill? When am I responsible for my bill?

If you have given us complete insurance information and we are in network with your insurance, you will not receive a bill until your insurance:

  • denies the claim
  • paid the claim and there is a co-insurance, deductible, or non-covered service for which you are responsible 
  • notifies us that information is needed from patient before claim can be processed
  • determines your coverage is not valid or cannot be verified for the date the services were provided

Payment of your bill is ultimately your responsibility and due upon receipt of your first statement.

What is a referral or authorization?

Some insurance plans require a referral when seeking care from a specialist. A referral is completed by your primary care physician and is sent into your insurance company. A referral means that your primary care physician has approved for you to see a doctor outside of your primary clinic. A referral DOES NOT guarantee payment from your insurance company

 

Neurosurgical Associates, Ltd will work with you and your insurance company to obtain an authorization for certain services. An authorization means that a proposed medical visit/treatment is medically reasonable and necessary. An authorization DOES NOT guarantee payment from your insurance company

When do I pay my co-pay?

Per your contract with your insurance company, your copay is due at the time of service. The amount is determined by your insurance plan.

What are my payment options?

You can pay in full with cash (at the time of your visit), check, or by credit card (Visa, MasterCard, American Express or Discover).

Neurosurgical Associates, Ltd participates with the following programs to help you arrange monthly payments:

  • MedCredit Financial Services offers financial loans similar to charge accounts. Advantages of opening a MedCredit account include immediate financial approval, no credit application, no annual fees or dues, and convenient monthly payments. Applications are available in our clinic. Annual interest rate of 8% applies to balances. 
  • Payment plans may be arranged with Neurosurgical Associates for balances over $300.00.  Minimum monthly payment of 10% per month is required.

What is included in Neurosurgical Associates fee for surgery? What is a global period?

Surgical fees include the actual surgery and any office visits (with our providers) related to uncomplicated follow-up care for the surgery within the global period.  The surgical global period does not include post operative complications (that require a return trip to the operating room) or services not performed by our providers (i.e., x-rays, labs, physical therapy, CT's, MRI's). 

A global period is a time limit of either 10 or 90 days that is assigned to a surgical procedure by your insurance company.  Most major surgeries have a global period of 90 days.
