When you arrive for your first appointment, we will review your medical records, X-rays, magnetic resonance imaging (MRI), computerized tomography (CT scan), myelograms, electromyelogram (EMG) and any other testing you may have had done. It is very important that you either bring this medical information with you at the time of your appointment or have it sent to Neurosurgical Associates at least a week before your scheduled visit. If your medical information is not available at the time you arrive, we may have to reschedule your appointment.

To ensure the highest level of care at your appointment, please make note of the following:

• Please complete the appropriate forms from our Patient Forms page and bring them with you to the appointment.
• Bring all insurance information with you -- we will need to make a copy of your insurance card(s). If it is work related or auto accident related, please make sure that you have the insurance companies name, address, claim number, adjuster name and phone number with you.
• Please be prepared to pay any co-payments which are due at the time of your visit. We accept cash, check, Visa and Mastercard.
• It is important to bring all recent x-ray films and reports to your appointment. Failure to do so will require us to reschedule your appointment.
• If your insurance plan requires a referral, please contact your primary care physician's business office to verify that a referral has been issued.
